Safe transitions of care rely on structured handoff tools such as SBAR (Situation, Background, Assessment, Recommendation), which should be used for shift handoffs, transfers between units, and calls to providers. Discharge planning begins at admission, not the day of discharge, and should involve case management, social work, physical/occupational therapy, and the patient's support system early.
