Therapeutic Communication & Milieu Management
Therapeutic communication techniques create space for a patient to express themselves and feel heard: open-ended questions ("Tell me more about that" rather than a yes/no question), reflection (mirroring back what the patient said or seems to feel), silence (giving the patient time to gather thoughts without rushing to fill the space), and offering self ("I'll sit with you for a while") all build trust and encourage disclosure. Non-therapeutic responses — asking "why" (which can sound accusatory and put a patient on the defensive), giving false reassurance ("everything will be fine"), giving unsolicited advice, changing the subject, or agreeing/disagreeing with a delusion — shut down communication or reinforce something inaccurate, and recognizing which category a given response falls into is one of the most heavily tested skills in psychiatric nursing content.
