Before evaluating answer choices, read the stem twice and identify exactly what it's asking: is it asking for the first action, the priority action, or the action that requires further teaching? These three question stems can point to completely different correct answers on the exact same clinical scenario, and misreading which one is being asked is one of the most common, entirely avoidable sources of missed points.
