Hemodynamic Monitoring Basics

Invasive hemodynamic monitoring is used in critically ill patients when frequent, precise, real-time data is needed beyond what a standard blood pressure cuff and pulse oximeter can provide. An arterial line gives continuous, beat-to-beat blood pressure readings and easy access for frequent arterial blood gas sampling — nursing priorities include ensuring the transducer is leveled and zeroed at the phlebostatic axis (roughly the level of the right atrium, found at the fourth intercostal space, mid-axillary line) for accurate readings, checking the insertion site and extremity for adequate perfusion (since an arterial line can compromise distal circulation), and never using an arterial line for medication administration, since arterial injection of many medications can cause severe tissue damage.
