Medication Calculation & the Rights of Administration

The traditional "Rights" of medication administration — right patient (two identifiers), right drug, right dose, right route, right time — are commonly extended to include right documentation, right reason, and right to refuse. Dosage calculation questions typically use the formula: Desired dose ÷ Available dose × Quantity = Amount to give, and weight-based dosing (especially pediatric and high-alert drugs like heparin) requires converting to kilograms first.
