Pediatric Medication Safety & Weight-Based Dosing
Most pediatric medications are dosed by weight (commonly mg/kg), which means a dosing error has much less room for error than in adult care — a small miscalculation can represent a large proportional overdose in a small child. The nurse's responsibility is to independently calculate the expected dose from the child's weight in kilograms and the ordered mg/kg dose, then compare that calculated dose against what's actually been ordered, rather than assuming the prescribed dose is correct; many facilities also require a independent second-nurse verification for high-alert pediatric medications before administration. Weight should be obtained in kilograms directly (not converted from a remembered pounds figure) and updated at each visit or admission, since dosing based on outdated or estimated weight is a recognized source of medication error.
